Set Up Needed - For set-up/take-down, a CUSTODIAL INFORMATION FORM and FLOOR PLANS (detailing table placement, chair placement, etc.) must be completed and returned to the church office no later than four (4) weeks before the event. Events requested for Saturdays must allow enough time for clean-up and set-up for Sunday morning; this includes sound, lighting, and recording.

For all Childcare requests, please pick up a Childcare Reqest Application from the church office.

If childcare is approved, a roster of each child detailing their name, birthdate/age, parent/guardian, numbers, allergies, etc., must be submitted to the church office no later than two (2) weeks before the event is to take place. This is to ensure the ratio of workers to children in addition to providing workers.
